Knowledge about Life-sustaining Treatment Decision Making, Attitudes toward Self-determination and Difficulty at Clinical Application in Doctors and Nurses

Abstract
This study is a descriptive research study to identify the knowledge of doctors and nurses, their attitude toward respect for self-determination of life-sustaining treatment, and difficulties in clinical applications regarding the ¡°Act on Decisions on Life-sustaining Treatment for Patients in Hospice and Palliative Care or at the End of Life¡±. A questionnaire survey was conducted on a total of 182 people, 88 doctors and 94 nurses. The main findings were as follows; 1) In the knowledge of Life-sustaining Treatment Decisions Act, systematic and repetitive education is necessary because of a low understanding about the detailed clauses and the role of the Medical Institution¡¯s Ethics Committee that can cause legal and ethical problems. 2) The attitude toward self-determination of life-sustaining treatment has been positive and puts emphasis on self-determination, but the family was still at the center of the decision. Therefore, it is important to educate the medical staff's firm attitude and belief in self-determination of life-sustaining treatment so that the enhancement of autonomy is realized in the medical field. 3) The most difficult part of clinical applications was the procedural complexity and the lack of time. This should be supported by medical institutions, such as strengthening the role of the medical institution ethics committee and securing dedicated personnel. Based on this survey, educations for medical staff could increase their knowledge of Life-sustaining Treatment Decisions Act, and the higher the knowledge and the higher the number of education, the more positive the attitude toward the respect for the self-determination of life-sustaining treatment. In addition, continuous education is considered to be important as the education of Life-sustaining Treatment Decisions Act and the preparation of legal forms can reduce the difficulties experienced in clinical application.
